Deadline for CPAC documents THIS FRIDAY!

FYI - I was just now looking over some of the many documents we need to fill out before the assessment and I realized that they are due this Friday. Gripes!...

We ask that you complete the enrollment documents as soon as possible, and not later than Friday, October 13, 2006, The completed Enrollment Packet should be emailed to pliddell@potterswillconsulting.com with a copy to befoulke@aol.com. You must also send by US Mail, postmarked not later than Friday, October 13, 2006, or fax your completed Millon III Psychological Inventory, Spiritual Gifts Inventory and Confidentiality and Fee Agreement.
